2023-2024 Kindergarten & Primary Screening Service Application Form

Notice for KPSS

  1. Parent has to submit the application form along with copies of previous assessment reports, if any, related to the student’s learning or developmental issues.
  2. Please be punctual. Late comer who cannot finish all items in the screening test has to make another appointment to finish the test, so that teacher can have enough information to complete the screening evaluation summary.
  3. Student has to bring along the following documents:
    • Any related assessment reports (if any)
    • Homework handbook (recent year)
    • KG student: academic report (recent year)
    • Primary student: Chi / Eng / Maths homework, test and exam papers (recent year)
    • Documents are for reference only. Copies will be made if necessary.
    • If the above documents cannot be provided, you have to submit later for us to write the evaluation summary.
  4. About 14 working days are required to complete the screening evaluation summary
